Transitional Care Unit Readies Patients for Home
 Lapeer Regional Medical Center’s Transitional Care Unit is a link between hospital and home, an added level of care available within the hospital.
The unit is for people who are well enough to leave the hospital but who are still unable to fully care for themselves at home. The state of Michigan nursing home regulatory department licenses the unit.
Relaxed setting
Residents of the Transitional Care Unit are encouraged to be up and dressed, to participate in activities and to socialize in the multi-purpose room. Visitors are welcome.
The focus in the TCU is a team approach to meeting each individual’s needs. Staff members consider the physical, emotional, and social needs that need to be addressed so that the person can make a smooth transition home.
Team members of the TCU include: the resident, resident’s family, resident’s physician, nurses, social worker, physical therapist, occupational therapist, speech therapist, dietitian, and recreational therapist.
Security and Convenience All in One
Because the Transitional Care Unit is located within the hospital, the TCU utilizes the hospital’s lab, diagnostic imaging, respiratory therapy, pharmacy, and support departments. This close proximity provides for immediate access for medical emergencies. The physician, case managers and TCU admission coordinator determine eligibility for the TCU.
To be accepted into the TCU, a person must be mentally alert, have a desire and willingness to participate, have a diagnosis that predicts improvement and have appropriate financial coverage.
